Use this to record the results of your risk assessment

You can use this form to help you complete a risk assessment. All employers must conduct risk assessments for their work activities. If you have five or more employees, you must record the significant findings of the risk assessment and bring them to the attention of those at risk.

Use a risk assessment to identify the need for further, more specific assessments such as fire, hazardous substances and display screen equipment. These have their own requirements. You can find information on them in our safety and health risks sections.​

On the form, you will need to say what further control measures you will put in place if you are not already doing enough to guarantee the safety and health of staff and other people. If you are unsure about controls, please check our guidance on different safety and health risks or contact our free adviceline on 0800 019 2211. Calls are free and completely confidential.

You will be asked to provide the name of the person responsible for leading on further controls and say when they will be in place. Please make sure you allocate the tasks to the right person or department, and ensure they have enough resources to carry them out. Delegating responsibility to someone else is not always enough if you have overall responsibility for the control measure - you will need to check whether the task is completed.
